The temple is significant as one of the five Pancha Bhoota Sthalams of Shiva, representing water, and as a Paadal Petra Sthalam praised in the Tevaram hymns. It is one of the most important Shaiva temples in the Tiruchirappalli region and is closely associated with the worship of Shiva as Jambukeswarar and Goddess Akilandeswari.

The temple is celebrated in the Tevaram hymns of the Tamil Saiva Nayanmars, which gives it the status of a Paadal Petra Sthalam. Traditional and historical accounts associate the temple with the Chola king Ko Chenganan. The temple has a long history of South Indian royal patronage and later structural additions, especially within the Tamil Shaiva temple tradition.

According to the temple's sthala purana, Goddess Parvati, worshipped here as Akilandeswari, performed penance and worshipped Shiva at this site under a Jambu tree. The lingam is associated with water, reflecting the temple's identity as the Jala or Appu Sthalam among the Pancha Bhoota Sthalams. Another temple tradition connects the site with the worship of Shiva by an elephant and a spider, a legend associated with the name Thiruvanaikaval.
